Feedback forms, and letting people claim their certificate

On the Feedback tab, build a short form or start from the Event feedback or CME evaluation template. People answer it on their pass. To make a certificate wait for it, tick "Gave feedback" for that role.

Answers are anonymous unless you choose otherwise: you see who finished the form, but not who said what.

With self-claim on (Attendance tab), people who qualify confirm how their name should appear and get their signed certificate on their pass straight away. Small spelling fixes are made on the spot; bigger changes come to you to approve first.
